Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is an evidenced-based recovery model designed to provide treatment, rehabilitation, and support services to individuals diagnosed with a serious and persistent mental illness whose needs have not been met by more traditional mental health services. Our ACT teams are interdisciplinary and include specialists in the areas of psychiatry, mental health, drug and alcohol, vocational training, housing, and nursing. Services are designed to increase individual success and satisfaction with employment, education, and living environments. Services are provided 24/7, 365 days/year.

CRISP

The CRISP (ACT+) program is an “outside the box” intensive outpatient mental health program for adults with serious mental illness for whom traditional services have proved unsuccessful. The CRISP model allows for individualization of services to a degree not typically available. CRISP receives all of our referrals directly from the EEU (Eligibility and Enrollment Unit) at DSAMH (Division of Substance Abuse and Mental Health) following a Level of Care assessment by Promise determining a CRISP Level of Care recommendation.”

Peer Support

PROMISE Peer Services is a program for qualifying beneficiaries to receive peer support from a peer support specialist. The Peer Recovery Specialist (PRS) is a person who has lived experience with mental and/or substance use disorder. S/he is responsible for providing peer support to other assigned individuals who are experiencing mental and/or substance use disorder and need support to live their best lives. Peer Recovery Specialists provide advocacy, insight, and support to other team members and service recipients about the successes and challenges faced by individuals who are living with mental/substance use disorder. PROMISE Peer Specialists will work on the goal/s that the beneficiary and PROMISE Care Manager come up with together. Each client has a set amount of time allotted to spend with the peer each week to which the PROMISE Peer wil meet. Some goals could include things like “increase community integration” for example. In this case, the Peer Specialist would assist beneficiary out in the community to find healthy activities such as volunteering, or going to the library, park, grocery stores, etc.

Mental Health Group Homes

Working collaboratively with the Division of Substance Abuse and Mental Health, Conexio Care, Inc provides comprehensive care to adult individuals with severe and persistent mental illness. As of December 2023, Conexio manages nine group homes throughout the state and those group homes house five to ten individuals per home. Our in-home teams are comprised of psychiatric and nursing professionals along with clinicians, certified nursing assistants and direct support professionals. We are committed to a person-centered approach that is designed to meet the unique needs of all individuals supported.
