As a grantee of the Delaware Continuum of Care, Conexio Care is the recipient of 11 U.S. Department of Housing and Urban Development (HUD) grants. These grants are use to provide Permanent Supportive Housing to chronically homeless persons or families within Delaware.
Permanent Supportive Housing serves over 600 chronically homeless households statewide each year. Housing is offered at reduced rents to low-income tenants. Case managers identify and link qualified tenants to permanent housing, mainstream resources, and develop a tenant led strength-based housing stability plan. Tenants are offered education regarding harm reduction, landlord tenancy rights and are engaged with motivational interviewing.
